Turn Your Hygiene Department Into Your Highest-Margin Revenue Engine

The P3 Hygiene Growth System™ is a 6-month consulting engagement that helps general practices and DSOs grow hygiene revenue 25–40% — strengthening EBITDA and adding six to seven figures of practice value.

Dr. Joy Void-Holmes, DHS, RDH — 29 years in dentistry · Licensed RDH since 1997 · Creator of the P3 Hygiene Growth System™

Most Practices Are Leaving Six Figures on the Table — In One Department

The math is simple, and most doctors know it intuitively: hygiene should be running 30–35% of total practice production. Many practices run at 22–28%. The gap is real money, every month, for years.

It isn’t a clinical skill problem. Your hygienists are competent. The problem is structural:

  • No standardized clinical protocols, so production varies by hygienist
  • Inconsistent perio diagnosis, leaving D4910 and D4341 revenue uncaptured
  • Recare retention drops because the system depends on memory, not workflow
  • Adjunctive procedures (sealants, SDF, fluoride) treated as optional add-ons instead of standard care
  • KPIs that exist on paper but never reach the daily huddle.

When hygiene runs without a system, you don’t notice the leak. You feel it slowly — flat production, frustrated team, lower offers when the practice is appraised. The P3 Hygiene Growth System™ closes the gap on purpose.

The P3 Framework – Protocols , People, Performance

P3 is built on the principle that hygiene production is a system output, not a personality trait. Every high-performing department I’ve worked with shares the same three pillars in alignment.

P

Protocol

Evidence-based clinical standards, accurate perio coding and documentation, integration of technology (air polishing, ultrasonics, SDF), and standardized workflows that protect clinical quality while increasing predictable production.

P

People

Hiring profiles for high-performing hygienists, structured onboarding, leadership development for hygiene coordinators or lead RDHs, and team alignment around the why behind growth metrics so execution sustains itself.

P

Performance

Clear KPIs tied to revenue, profit, and retention. Monthly dashboards. Recare and perio tracking systems. Metrics that align with EBITDA and valuation multiples — not just clean teeth.

Case Study: From Zero Hygiene Days to 40% of Practice Production

        Single-doctor general practice · 2-year P3 engagement.

The starting point. The owner-doctor was doing his own hygiene and relying on temps to cover chairs when he couldn’t. He had never hired a permanent hygienist. There was no hygiene system, no recare strategy, and no way to grow beyond the doctor’s own chair time.

The intervention. A 2-year P3 implementation covering the full hygiene build-out — hiring, onboarding, clinical protocols, perio diagnosis standards, recare systems, KPIs, and team training.

        The results at engaement completion:

What lasted. After the engagement ended, the practice continued to perform on the systems we built. Sustained excellence — not a temporary lift.

        What this case proves:

Hygiene production is a system output — not a personality trait. With the right protocols and the right people, the numbers follow. The methodology behind this case has been refined for over two decades — the principles you see here are the same principles in every P3 engagement today. The question isn’t whether your hygiene department can hit 30–35% of production. The question is whether you have the system in place to make it happen.

What Happens When Hygiene Teams Bring P3 Back to Their Practices

“After attending the P3 Workshop, our team implemented [specific protocol change] in our practice. Within [timeframe], our [specific KPI — e.g., perio diagnosis percentage] went from [starting number] to [ending number]. Our doctor said it was the best ROI we’ve ever seen on team training.”

        The P3 Production Guarantee

You take real risk when you commit to a 6-month consulting engagement. So do I.

If your hygiene department hasn’t grown production by at least 15% — or $40,000, whichever comes first — within 6 months of P3 implementation, I continue working with your team at no additional cost until the result is delivered.

Conditions: practice must have implemented the protocols, attended scheduled calls, and submitted KPI data as outlined in the engagement. The work goes both ways.

This isn’t a marketing line. It’s how I structure every P3 engagement. My incentive is your hygiene production. If we don’t hit it, I haven’t finished the job.

Why I Cap My Engagements — and Why Now Matters

P3 Complete is a high-touch consulting engagement. To deliver the level of strategy, training, and on-site support included, I limit capacity intentionally.

I work with no more than 6 active practice engagements at any one time — to ensure every client gets the depth of strategic support, on-site presence, and async availability the system requires.

A note on transparency. P3 has trained thousands of hygienists since 2022 and refined the underlying methodology for over 20 years. The 6-month consulting engagement — P3 Complete — is the newest expression of that work, designed to give practice owners direct access to the full system. Founding clients receive my full, undivided attention as I build the consulting arm of P3 to scale. The methodology is proven. The consulting format is being built with you, not before you.

Hygiene Production Benchmarks — The Numbers I Reference Most

What should hygiene production contribute to total practice revenue?

A healthy general dentistry practice runs hygiene at 30–35% of total production. Below 28% almost always indicates a system gap — either in protocols, scheduling, or perio diagnosis. The number itself is less important than the trend: if hygiene as a percentage of production is flat or declining year over year, something is leaking.

How much should each hygienist produce annually?

Industry benchmarks place a healthy hygienist at $250,000–$400,000 in annual production, depending on geography, patient mix, and adjunctive procedure adoption. Hygienists practicing without standardized protocols often run 20–30% below that range — not because of skill, but because of system inconsistency.

What should production per hygiene hour look like?

$200–$300 per hour is healthy. Below $150 indicates an undertreatment pattern — usually missed perio, missed adjunctive procedures, or coding inaccuracy. Above $300 is achievable in practices with strong air polishing, SDF, and ultrasonic protocols in place.

What should the average production per hygiene visit be?

$200–$300 per appointment is the healthy range. The variance comes from adjunctive procedure adoption — practices that consistently incorporate fluoride, sealants, SDF, and air polishing land in the upper half of that range. Practices that only bill prophy and exam land in the lower half.

What should my periodontal procedure mix percentage be?

In a typical adult patient population, 25–35% of recall patients should be on perio maintenance (D4910). Practices running below 15% are almost always under-diagnosing perio — either through inconsistent probing, charting that isn’t reviewed by the doctor, or hesitation around treatment presentation. This is one of the highest-leverage gaps to close.
